United Healthcare denying claims: what to do when the denial is yours
If United Healthcare denied your claim, you generally have 180 days to appeal (29 CFR 2560.503-1(h)(3)(i)), then four months to ask for independent external review.
Put in the date on your UnitedHealthcare notice. You get both federal deadlines as calendar dates, and the body that decides your case at the end.
Free, for a United Healthcare denial, and it runs entirely in your browser. The date, the plan type and the state you pick are never sent to us and never stored.
The date runs from the day printed on the notice. A Medicare Advantage plan from UnitedHealthcare runs on Medicare's clocks instead; that route is linked below.
Or let the $29 Appeal Kit date each step and write the claim-file and external-review requests.
United keeps denying my claims. I'm up to my ears in medical debt and I make close to nothing.
A denial from UnitedHealthcare is the start of a process, not the end of one. Federal law gives you two free stages: an appeal to UnitedHealthcare itself, then a review by an organization UnitedHealthcare does not control, whose decision binds the plan. Nationally 32.6% of internal appeals and 32.8% of external reviews in the federal file were overturned for the people who filed them. Those are outcomes for filers, not a prediction about your claim.
If United Healthcare denied a claim of yours, these two dates matter more than any percentage. You have 180 days from the date on your denial notice to file the internal appeal, and then 4 months from the final internal denial to ask for an independent external review. If waiting would seriously jeopardize your health you can ask for both to be expedited, decided in about 72 hours. Both timeframes are federal, not your insurer's choice, 29 CFR 2560.503-1 (PDF) and 45 CFR 147.136 (PDF), in plain English at HealthCare.gov, and your own denial letter states the ones that govern your plan. The PDFs are the same sections on govinfo.gov, which serves them without a challenge page, so an automated reader can check these two dates as easily as you can.
United Healthcare denied claims: what the federal file shows
UnitedHealthcare's 15 reporting entities denied 7,137,191 of 37,134,878 in-network claims (19.2%) in the plan-year 2026 CMS file. That rate, entity by entity, is on United Healthcare denial rate. This page is about what to do with your own denial.
The reason categories the file records most for UnitedHealthcare: Provider out of network (3,311,785), Other reasons (1,798,006), Service excluded from the plan (1,010,725). The reason printed on your letter is the one that decides your argument.
What people did next, record by record. A blank in the federal file stays a blank here; we never read a missing figure as a zero.

How UnitedHealthcare says an appeal works, in its own words
We quote each line as written and label it member or provider. A provider-side clock is never your clock as a member. Member clocks can differ by plan and by state, and your denial letter is the authority for yours.
- Member process. Member filing window: 180 calendar days. “within 180 calendar days following any incident or action that is the subject of your dissatisfaction” Note: Member-side second source. UnitedHealthcare's provider page publishes no member window; this member form states 180 days. From Member service request form (memberforms.uhc.com, carries state-specific sections incl. California IMR; undated), checked August 26, 2026.
- Member process. Who may appeal (member side): member, authorized representative, or contracted provider. “a timely request by a member, an authorized representative or a contracted provider to change a plan denial decision” From Member service request form (memberforms.uhc.com, carries state-specific sections incl. California IMR; undated), checked August 26, 2026.
- Member process. Member review clocks: acknowledge 5 days, decide 30 days standard / 3 days urgent. “acknowledge receipt within 5 calendar days and provide an answer within 30 calendar days for a standard review. If your problem is urgent, UnitedHealthcare must give you a decision within 3 calendar days.” Note: Stated on a form page that also carries California-specific sections (DMHC 1-888-466-2219, Independent Medical Review), plan-state specifics can vary. From Member service request form (memberforms.uhc.com, carries state-specific sections incl. California IMR; undated), checked August 26, 2026.
- Provider process. Combined window for both steps: 12 months. “You have 12 months to complete the following steps: Step 1: File a claim reconsideration request. Step 2: File an appeal if you disagree with the outcome of the claim reconsideration decision.” Note: One clock covers both steps, a slow reconsideration eats the appeal window. From Pre- and post-service appeals and reconsiderations, checked August 25, 2026.
- Provider process. Two-step structure is mandatory: reconsideration, then appeal. “you must submit a claim reconsideration request. If you don't agree with the outcome of the reconsideration, you may submit an appeal” From Pre- and post-service appeals and reconsiderations, checked August 25, 2026.
- Provider process. Assignment of benefits is not representation: separate patient consent needed. “an assignment of benefits does not constitute designation of an authorized representative” Note: For patients: your provider billing UnitedHealthcare for you does NOT automatically let them appeal AS you, a separate authorization is needed, or you appeal yourself. From Pre- and post-service appeals and reconsiderations, checked August 25, 2026.
- Provider process. Submission is portal/API: UnitedHealthcare Provider Portal or API. “You can use either the UnitedHealthcare Provider Portal or an API … to submit a post-service appeal.” Note: No mailing address or fax is published on this page. From Pre- and post-service appeals and reconsiderations, checked August 25, 2026.
- Provider process. Expedited appeal is pre-service only: not available after the service. “If you have already provided the service, an expedited or urgent appeal is not available.” From Pre- and post-service appeals and reconsiderations, checked August 25, 2026.
- Provider process. Peer-to-peer review windows: 3 business days inpatient / 21 calendar days outpatient. “Inpatient cases must be submitted within 3 business days and outpatient cases within 21 calendar days from posted denial.” From Pre- and post-service appeals and reconsiderations, checked August 25, 2026.

Your two dates, and what each one opens
| Stage | Your time | The rule |
|---|---|---|
| Internal appeal to UnitedHealthcare | At least 180 days from the denial notice | 29 CFR 2560.503-1(h)(3)(i) |
| External review by an independent organization | Four months after the final internal denial | 45 CFR 147.136(c)(2)(vi) and (d)(2)(i) |
Ask for the claim file in the same letter. You may have it free of charge under 29 CFR 2560.503-1(h)(2)(iii), and it shows you what UnitedHealthcare relied on.
If UnitedHealthcare runs your Medicare Advantage plan
The dates above are for plans through a job or the Marketplace. A Medicare Advantage plan has its own first step: a plan reconsideration, asked for within 65 days from the date on the plan's notice, according to Medicare.gov. The whole route is on appeal a Medicare denial.
When the free stages are used up
The time limit to sue is not one federal number. On a plan through a job, it is often written into the plan itself. The statute of limitations on insurance claims sets out all three clocks, and what an attorney adds at each stage covers the rest.
Questions
Why is United Healthcare denying claims?
The federal file records a reason category for each denial. Across UnitedHealthcare's reporting entities the largest categories were provider out of network (3,311,785), other reasons (1,798,006), service excluded from the plan (1,010,725). Your own letter has to state the specific reason and the plan provision it relies on, under 29 CFR 2560.503-1(g)(1)(i) and (ii). That sentence decides which argument fits your appeal.
What percentage of United Healthcare claims are denied?
19.2% of in-network claims across UnitedHealthcare's 15 reporting entities in the plan-year 2026 CMS file, 7,137,191 of 37,134,878. The brand page breaks that rate down entity by entity. A rate describes a book of business, not your claim.
What phone number do I call about a denied United Healthcare claim?
Use the member number on your UnitedHealthcare ID card or on the denial letter itself; we print no UnitedHealthcare number we have not read at source. If the plan comes through a job, the U.S. Department of Labor answers free on 1-866-444-3272.
Can my doctor appeal for me?
UnitedHealthcare's member form says an appeal can come from the member, an authorized representative or a contracted provider. Its provider page adds that an assignment of benefits is not the same as naming your provider as your representative. If you want your provider to act for you, sign the authorization; otherwise file yourself.
Two dates decide more of this than anything else, and neither is on the front of your letter in a way anyone notices. Work them out tonight, write them on the letter itself, and you have already done the part that is easiest to lose. If your dates are strange, a denial that arrived after the treatment, or a letter with no external-review instructions on it at all, send us the wording with the identifying details removed and we will tell you which clock you are on.

Counts: CMS Transparency in Coverage Public Use File (PY2026 release, PY2024 claims), data.healthcare.gov — US Government public domain. Rights and timeframes: 29 CFR 2560.503-1 (180 days, and your free right to the claim file) and 45 CFR 147.136 (4 months, and what qualifies), explained at HealthCare.gov. Expedited review is decided in about 72 hours under both sections.
